Please scan and upload the following documents:

- Form B (PDF, max. 1 MB)
- Form C (PDF, max. 1 MB)
- Birth Certificate (PDF, max. 1 MB)
- National ID / Driver’s License (PDF, max. 1 MB)
- High School Diploma (PDF, max. 1 MB)
- Undergraduate Degree Certificate (PDF, max. 1 MB)
- UKMPPD Graduation Certificate (PDF, max. 10 MB)
- Undergraduate Academic Transcript (PDF, max. 1 MB)
- National Symposium Committee Certificate (PDF, max. 1 MB)
- UMS Faculty of Medicine Library Clearance Letter (PDF, max. 1 MB)
- Color ID Photo with Blue-DOP Background (JPG/JPEG, max. 10 MB)
Completing the Registration Form
Your registration form PDF will be automatically generated once you have submitted the requirements form. Download the file and print it on A4 paper.
Document Submission
- Arrange all physical documents in order according to the checklist.
- Place them inside a YELLOW ENVELOPE.
- Write your personal details on the front of the envelope.
- Submit the documents to the Medical Professional Program office at our Faculty of Medicine.
- If you have not received an email reply, please follow up directly with the Medical Professional Program office.
